A diverse variety of arrhythmias appear early and may worsen the course of ACM, atrial fibrillation being the most frequent 60 and ventricular tachycardia the most deleterious 61.

What are the symptoms of alcoholic cardiomyopathy?

This can occur through direct means, by promoting the generation of free radicals, or indirectly, by triggering the release of hormones, such as angiotensin II, or activating other systems. Moreover, alcohol may reduce the levels of transport proteins and diminish antioxidant activity by decreasing the plasma concentration of antioxidant enzymes. These mechanisms contribute to the development of oxidative stress, which is responsible for the onset of cardiomyopathies and ischemia-reperfusion injury. Using a C56BL/6J mouse model, Hu et al. investigated the effects of chronic ethanol consumption on long-chain fatty acid (LCFA) uptake, LCFA gene expression (e.g., genes/proteins involved in esterification of triglycerides), LCFA transporters, and cardiac structure and function (33). Animals received different concentrations of ethanol in their drinking water (10%, 14%, 18% v/v) for variable weeks (12, 8, and 4, respectively). In all three ethanol groups, compared to control groups there was a significant increase in heart weight-to-body weight ratios.

Treatment / Management

We reviewed the effects of ethanol on the cardiovascular system in 1996 15, including aspects of inflammation 16, rhythm disturbances 17, and hypertension 18. In 2001 we updated the data on the ambivalent relationship between alcohol and the heart 19 and in 2008 added new evidence on a larger cohort of patients with different forms of cardiomyopathy and increased alcohol intake from the German competence network on heart failure 20. Regarding ICD and CRT implantation, the same criteria as in DCM are used in ACM, although it is known that excessive alcohol intake is specifically linked to ventricular arrhythmia and sudden cardiac death71. As it is not uncommon in ACM for patients to experience a significant recovery of systolic function, it is particularly challenging in this disease to decide the most appropriate time to implant an ICD and whether it is necessary to replace a previously implanted device. Future studies in ACM should also address this topic, which has important economic consequences. This is because the ethanol molecule has a small size and is highly reactive, with many cell targets.

Renaud and de Lorgeril 93 suggested that the inhibition of platelet reactivity by wine may be one explanation for protection from CAD in France. Alterations caused by heavy alcohol intake have also been studied from the perspective of histopathology. Emmanuel Rubin analysed muscle biopsies from individuals who were previously non-drinkers and were submitted to a balanced diet with heavy alcohol intake during one month41. Although no significant changes were found using conventional microscopy, when electron microscopy was employed he discovered intracellular swelling, glycogen and lipid accumulation, and alterations in the structure of the sarcoplasmic reticulum and of the mitochondria (Figure 2).

Polyphenols of red barrique wines and flavonoids have been shown to inhibit endothelin-1 synthase 102 and PDGF-induced vasoproliferation thus also contributing to cardiovascular protection 103. Many people end up in urgent care or the emergency room because of the early symptoms of alcoholic ketoacidosis, which include abdominal pain. If your ketone levels get too high, you might start suffering from alcoholic ketoacidosis, also known as alcoholic ketosis. It should be noted that ketoacidosis is very rare9 and not a significant risk factor for AKA unless someone is also chronically abusing alcohol. Following resuscitation, our patient had plasma electrolyte levels corrected, nutritional supplementation provided and completed an alcohol detoxification regimen. Given the early recognition of AKA and concurrent management, our patient had a good outcome.

Although the cooccurrence of depression and alcohol use disorders has been confirmed by several studies, the relationship between the two disorders has been difficult to describe 9. Studies have attempted to differentiate between depressed and nondepressed alcohol-dependent persons with particular focus on the participant’s level of alcohol dependence, demographic characteristics, or illness-related variables. It has been shown that depression is more related to the current alcohol drinking episode than lifetime diagnosis of depression 11. Depression diagnosed in the current episode of alcohol dependence normally remits after 2 weeks of detoxification and abstinence and falls to normal range within 3 weeks 11, 12. The rapid recovery is in contrast to the slower (17 weeks) recovery from a major depression 13.

Early on, the underlying etiology of a psychosis may be uncertain. During withdrawal from heavy drinking, people may develop delirium tremens, a complication of withdrawal marked by psychotic symptoms, such as hallucinations (see Core article on AUD). The prevalence of depression at intake before detoxification was 63.8% (120 participants). Three participants were referred for treatment of major depression within the period of the study.

The causal effect of AUD leading to depression implies that some cases of depression resolve after treatment of alcohol dependence 12, 30–33. Flensborg-Madsen et al. in a prospective study showed that the causal role of alcohol use disorders in major depression was stronger than the causal role of major depression in alcohol use disorder 16.

For many individuals struggling with depression, alcohol can seem like an easy, accessible form of relief. Its initial effects, which include relaxation and mood elevation due to increased dopamine and GABA activity, might provide a temporary reprieve from feelings of sadness, hopelessness, or anxiety. But with the right treatment and support, most people with depression can make a full recovery. When psychosis is suspected, a general physical and neurological exam should be performed to exclude medical causes such as subdural hematoma, seizures, or hepatic encephalopathy—any of which may be a consequence of AUD. Again, it’s important to create a timeline of mental health symptoms and alcohol use and to collaborate as needed with mental health specialists for selection of pharmacotherapies and psychosocial interventions. PTSD is characterized primarily by alterations in arousal and recurrent intrusive thoughts that follow a traumatic event.

As hangover symptoms begin to subside, the emotional effects may follow. Recovering from depression and AUD is difficult because the disorders can worsen one another. Often, people turn to alcohol to help relieve their depression symptoms. Although the study participant had a general physical examination done (including blood pressure, temperature, and body weight check), no laboratory or radiological investigations were done in the current study.
